Peripheral Nerve Stimulation (PNS) for Targeted Focal Pain

At Monterey Pain & Spine Institute (integrated within Majestic MD Spa), we offer Peripheral Nerve Stimulation (PNS)—an advanced, drug-free neuromodulation treatment designed to target localized, chronic pain stemming from individual peripheral nerves.

If you suffer from chronic, isolated pain in your shoulder, knee, hip, foot, ankle, or post-surgical incision site that has not responded to physical therapy or targeted injections, PNS provides a minimally invasive, surgery-sparing solution to relieve pain at its exact source.

What Is Peripheral Nerve Stimulation (PNS)?

Unlike Spinal Cord Stimulation (SCS), which acts on nerves within the central spinal canal, Peripheral Nerve Stimulation targets specific individual nerves outside the spinal cord.

Using high-resolution ultrasound or fluoroscopic guidance, a micro-thin, flexible wire (lead) is placed under the skin directly adjacent to the painful peripheral nerve. The lead delivers mild, high-frequency electrical pulses that disrupt pain signals sent along that specific nerve before they can reach your brain.

PNS can be used as a temporary 60-day therapeutic treatment designed to recondition nerve signaling, or as a long-term implantable solution for complex, persistent focal pain.

Conditions Treated with Peripheral Nerve Stimulation

PNS is highly effective for localized, nerve-specific pain conditions:

  • Chronic Shoulder Pain: Persistent nerve-related pain following rotator cuff repair, stroke, or joint wear.
  • Chronic Knee & Hip Pain: Post-surgical joint pain (e.g., persistent pain after total knee arthroplasty) or severe osteoarthritis pain.
  • Occipital Neuralgia & Severe Headaches: Persistent, shooting nerve pain originating at the base of the neck and radiating up the scalp.
  • Peripheral Nerve Injuries & Nerve Entrapment: Pain resulting from trauma, nerve stretch injuries, or prior surgeries.
  • Foot & Ankle Pain: Chronic nerve pain, including complex plantar fasciitis and nerve entrapments.

Benefits of Peripheral Nerve Stimulation

  • Targeted, Focal Relief: Precisely treats localized pain without affecting surrounding uninjured nerves or muscles.
  • Minimally Invasive Outpatient Procedure: Placed through a simple, needle-based approach using real-time imaging guidance—requiring no major surgery.
  • Reversible & Re-trainable: Temporary 60-day systems can reset painful nerve pathways without permanent hardware remaining in your body.
  • Reduces Medication Dependence: Provides significant, drug-free pain reduction, minimizing or eliminating reliance on prescription medications.
  • Insurance Coverage: PNS therapy is widely covered by Medicare and major commercial insurance carriers when clinically indicated.

What to Expect During the PNS Procedure

  1. Precision Imaging Guidance: Your physician uses high-resolution ultrasound to visualize the exact target nerve in real time.
  2. Micro-Lead Placement: Under local anesthesia, a micro-sized lead is gently introduced adjacent to the nerve.
  3. Comfort & Customization: The lead is connected to a small wearable transmitter. Your physician customize the pulse frequency to ensure optimal pain relief.
  4. Active Recovery: The procedure takes less than an hour, allowing you to return home the same day and resume light daily activities.

Synergistic Recovery Pathways

To support tissue repair and nerve health following your procedure, we integrate supportive recovery care:

  • Infrared Red Light Therapy to reduce localized tissue inflammation and encourage cellular healing.
  • Targeted Micronutrient & Peptide Protocols to support peripheral nerve regeneration and tissue repair.
  • Stay in the Game Pathway to help you safely restore joint range of motion and functional strength.
